If you have selected the Oxford Open option for your manuscript you are required to complete the following form in full, within five days of your manuscript being accepted for publication by Health Policy and Planning. If you have not selected the Oxford Open option please do not complete this form.

As a Corresponding author based at an institution that does not have an active online subscription to Health Policy and Planning you have opted to pay the full open access charge of 1500 GBP/3000 USD/2250 EURO. Special rates apply for Corresponding authors based in developing countries - you will need this information for section 7. Please check the list of qualifying countries before attempting to complete the form. Special rates may also apply for certain article types - please check section 7 below.
